Legacy Community Health Services will have 60 AmeriCorps members who will address barriers in accessing primary and preventative healthcare services by connecting patients and community members to resources and through individualized patient navigation services throughout the Greater Houston area. At the end of the program year, the AmeriCorps members will be responsible for assisting 9,360 low-income and medically underserved individuals. In addition, the AmeriCorps members will leverage 50 volunteers who will be engaged in addressing barriers in accessing healthcare. This program will focus on the AmeriCorps focus area of Healthy Futures. The AmeriCorps investment of $575,227 will be matched with $226,620 in private funding.
