Legacy Community Health Services (Legacy) will have 106 AmeriCorps members who will increase access to primary and preventative healthcare services by addressing social determinants of health for low income individuals throughout Harris and Jefferson counties. AmeriCorps members will be responsible for assisting 18,490 individuals, resulting in 11,095 individuals accessing primary healthcare services.
