Legacy Community Health will have 70 AmeriCorps members who will address barriers in accessing primary and preventative healthcare services by connecting patients and community members to resources and through individualized patient navigation services throughout the Greater Houston area. At the end of the third program year, the AmeriCorps members will be responsible for assisting 13,000 low-income and medically underserved individuals. In addition, the AmeriCorps members will leverage 10 volunteers who will be engaged in addressing barriers in accessing healthcare. The AmeriCorps investment will be matched with $317,268 in private funding.
