AccessHealth AmeriCorps members address and mitigate barriers to accessing basic needs and preventative health services by using a technological platform which links partners and community members to resources, promoting community health initiatives, and coordinating support for pharmacy programs to tackle social determinants of health in clinics in Fort Bend County. AmeriCorps members will serve 750 individuals to improve their access to healthcare.
